A DOT physical is required for commercial drivers to help confirm they meet federal health and safety standards. Our streamlined, in-person exam is designed to make the process efficient for drivers and convenient for employers who need their team certified quickly and back to work with minimal disruption.

Your exam includes a review of medical history, blood pressure, vision, hearing, urinalysis, and a physical evaluation. If you qualify, you may receive your medical certificate the same day and move forward without unnecessary delays.

Note: Complete the first two pages of the Medical Examination Report Form (MCSA-5875) before your exam.

What do I bring to a DOT physical?

• A complete list of all your prescription and over-the-counter medicines, including herbals, vitamins, supplements and essential oils.
• Medical records from any specialists you see.
• Visual and auditory aids like prescription glasses, contact lenses, and hearing aids that you use while driving. You’re allowed to wear these during vision and hearing assessments.
• If you have diabetes, you’ll need a record (at least three months of your electronic glucose monitoring records/readings and your most recent Hgb A1C.
• If you have sleep apnea, bring at least 90 days of data from your CPAP machine.
• If you have heart issues, you’ll need a letter from your cardiologist saying you’re safe to drive a DOT vehicle. Bring the results of a recent stress test, ECHOcardiogram, or other tests within the past 1-2 years.
• If you’ve had a stroke, brain tumor, seizure disorder, or bleeding in the brain, you’ll need a letter from your neurologist.
• If you take prescribed medications, do not skip or stop taking your medication before the appointment, as this could temporarily affect test results and your health.

Waivers/Exemptions for DOT Disqualifying Medical Conditions

Only medical examiners listed in the National Registry can certify drivers during DOT physicals, but they cannot approve or issue waivers or exemptions for disqualifying medical conditions. Waivers and exemptions—such as those for vision, hearing, and seizure disorders—are granted exclusively by the Federal Motor Carrier Safety Administration (FMCSA). Drivers must meet specific FMCSA criteria before submitting an application and all required documentation directly to the FMCSA for review and consideration.
